EPFO introduces composite claim form for death cases under the Employees’ Provident Funds and Miscellaneous Provisions Act, 1952

With a view to make its services available to the stakeholders in an efficient manner, the Employees’ Provident Fund Organisation (“EPFO”) has issued an Order dated 21st August, 2017 relating to claim forms for death cases under the Employees’ Provident Funds and Miscellaneous Provisions Act, 1952 (“Act”).

A new Composite Claim Form for Death Case (copy attached) has been introduced replacing the existing Forms No.20, 10-D and 5-IF. This claim form for death cases is effective from 21st August, 2017.

[Note:

  1. Form 20 was required to be submitted for claiming provident fund dues by a nominee or legal heir of the deceased member under the Employees’ Provident Fund Scheme, 1952.
  2. From 10D was required to be submitted for claiming monthly pension under the Employees’ Pension Scheme, 1995.
  3.  Form 5-IF was required to be submitted for payment of assurance benefit of deceased employee to nominee or legal heir under the Employees’ Deposit-Linked Insurance Scheme].
